I will send you an intake questionnaire to complete prior to our visit. It is important to complete as much of this form as you can so I have a better idea of what is bringing you to therapy and/or testing at this time.

CONSENT FORM & PRACTICE POLICIES

You will also receive a consent form prior to our first meeting. This form includes the policies and practices of my clinic. Please make sure to review the form carefully and contact me if you have any questions or concerns. You can find a copy of the consent form and practice policies here. A copy will also be sent to you prior to our first appointment.

RELEASE OF INFORMATION

If needed, a Release of Information can be downloaded here. I will work with your current or past provider(s) to coordinate care.
